There are a few things that we request to help expedite your visit. Please bring the following information to your scheduled appointment so that we may better serve your needs.

MEDICAL HISTORY:
Answer all the questions, if you have any significant medical conditions please inform us in advance. This allows us to contact your family physician prior to your visit and it will ensure your safety and comfort. If you take multiple medications please bring a list of those you are taking so that we may keep a copy in your chart.

INSURANCE INFORMATION:

The best way for us to maximize your Dental / Medical Benefits and help you, is to provide current and proper information. We need the following:

Insurance Company Name Billing Address and Phone Number Insurance Group Number (if available)

Insured’s SSN# Insured’s Employer Information

Referral Form and Xray:

If you were referred by your dentist, you may have been given a referral form and xray. If so, please bring what you have to your appointment.
